SERVICE AGREEMENT
____________authorizes Maritime Health Services to provide 24-hour emergency physician consultation service for the recreational vessel_______________.
____________agrees to use the service of Maritime Health Services, to abide by the terms and conditions stated herein and to pay all fees, charges and costs when due.
____________agrees to pay MariSafe, as agent for Maritime Health Services, a fee of $______ for coverage____________ effective__________. This fee does not include other optional services such as pharmaceuticals or medical supplies. This agreement is in force, subject to written notice of cancellation by either party with a 30-day notice.
____________agrees to pay for all communication charges incurred in the provision of care or advice. Maritime Health Services retains the option to monitor and record communications.

DISCLAIMER
Nothing contained in the Recreational Vessel Contract, nor the subsequent conduct or agreements of the parties shall create or be interpreted to create, the relationship of principal and agent, master and servant, or employer and employee.
The sole responsibility of Maritime Health Services, a division of Global Medical Systems, Inc., P.S. under the terms of the Service Agreement is to provide access to medical practitioners so that the subscriber may consult with such medical personnel. The authority and responsibility for each and every decision, action taken and omission, and each of them, remains within the sole and absolute discretion of the Master of subscriber’s vessel, and no liability of any kind shall be imputed to Maritime Health Services, by, or as a result of any such decision, act or omission of such Master.
Nothing contained in the Service Agreement shall in any way later, amend or change the terms and limitations of the Jones Act of the United States.
